MEDICAL INSURANCE INFORMATION
Medical Emergencies call 911 for immediate medical attention. If you are unsure if the situation
requires an emergency room visit and your personal physician practices at
one of the Kaiser Medical Centers, call the medical advice nurse 24 hours a
day at 800-677-1112. If your
physician practices in the community call the office directly: the number
is on the back of your Medical I.D. Card.

Dental Information
The Pressmen Welfare Fund has two dental plans.
Please make sure you know which plan you and your family enrolled
in! If you wish to change you can do
so only during the month of December each year.
To mail a dental claim for the Indemnity Dental Plan contact
the fund or Union office for a claim form and mail form and your itemized
bill from your dentist to:

You do not
mail any forms if you are enrolled in Group Dental Services, however you
must use a participating dentist. If
you need to find a participating dentist ask for a provider book from the
Union Office or from Group Dental Directly.

Your Prescription
Coverage is through Kaiser Permanente.
You have an option of picking up your prescriptions at
the Kaiser Pharmacies to optimize financial savings or using their mail
order. You can also use
participating community pharmacies in your area.
